The checkbox for accept COD de-selects automatically whenever you open your mailbox. There's no code to or feature to save the check marks. You have to manually click it each time as a security feature.
The only thing I can think of is that due to lag, or something else, the entire load process isn't completing, which is showing something erroneously. I'll move the un-check code higher up on the priority list, so you should never see it checked.
